College Information Forms are mandatory for all recipients.

Submission of these forms is NOT part of the application process. Only scholarship recipients submit. It is a final step required to receive your scholarship.

All BWEF scholarships to seniors and graduates fall into one of the two groups, below.

Deadlines for College Information Forms are different for each group.

It is critical that you study these deadlines carefully and submit your College Information Form on time. If your College Information Form is not submitted by the deadline, funds cannot be disbursed.

These forms along with photo release forms will be handed out to all non-Shidecker scholarship winners at Awards Night and are also available as Google docs or as hardcopy forms and/or as a QR code through the BWHS Guidance Office.

GRADUATING SENIOR SCHOLARSHIP WINNERS: We highly recommend that you provide a personal or parent’s email address to BWHS Guidance rather than your high school email address as the high school closes your email account shortly after graduation.


1. Shidecker Scholarship Award Winners:

The James T. and Lucille C. Shidecker Scholarship Fund is managed by The Columbus Foundation. After the BWEF selects scholarship recipients, the process of gathering College Information Forms and distributing funds accordingly is handled by The Columbus Foundation.

If you won a Shidecker Scholarship, you will receive a College Information Form from the Columbus Foundation either by mail or email. Be sure the BWHS Guidance Office has your permanent mailing and email addresses to share with The Columbus Foundation so they can deliver the form to you.

Complete The Columbus Foundation’s College Information Form by their deadline.

You will not have other deadlines or requirements from the BWEF after Awards Night.

Again, look for communication from The Columbus Foundation in early summer and return their form to them by their deadline.


2. Non-Shidecker Scholarship Award Winners:

If you earned a scholarship through the Big Walnut Education Foundation as a graduating senior or post-secondary student that is NOT a Shidecker scholarship, the process of gathering your College Information Forms and distributing funds accordingly is handled by the BWEF.

BWEF College Information Form 

These forms along with photo release forms will be handed out to all non-Shidecker scholarship winners at Awards Night and are also available as Google docs (see link above) or as hardcopy forms and/or a QR code through the Guidance Office.

BWHS Graduation Day at 11:59pm each year is the hard deadline for all scholarship award winners to submit their College Information Forms (excluding Shidecker winners; see above).

Again, without your College Information Form, funds cannot be distributed. If you fail to submit your College Information Form before 11:59pm on BWHS Graduation Day, you forfeit your scholarship and no funds will be distributed.

Any hardcopy college information forms mailed USPS with any risk of arriving after BWHS graduation or slid under the door of the Guidance Office with any risk of being received after graduation may not be received. The postmarked date does not apply in this situation. Again, forms must be in hand on or before 11:59pm on BWHS Graduation Day.


NOTES for Four-Year Award Winners:

If you earn a scholarship which will be distributed over four years, it will be funded by The James T. and Lucille C. Shidecker Scholarship Fund. Your annual requirement for the College Information/Verification Form submission will be handled by The Columbus Foundation.

The Columbus Foundation will contact you in early summer each year at the permanent mailing and/or email address that you shared with BWHS Guidance as a graduating senior. If your contact information changes, be sure to update it with The Columbus Foundation.
